Providing specialized in-home physiotherapy services to orthopedic, musculoskeletal, surgical, geriatric, MVA, and WSIB patients. This role involves comprehensive initial assessments, individualized treatment planning, and holistic care delivery focused on pain management, mobility enhancement, and functional rehabilitation. Key responsibilities include conducting ergonomic and home safety assessments, facilitating return-to-work transitions through collaboration with multidisciplinary teams (disability management specialists, prosthesis manufacturers, healthcare professionals), and managing detailed documentation for WSIB and insurance stakeholders.

• •Conduct comprehensive home-based assessments and develop individualized treatment plans for complex orthopedic and post-surgical cases
• •Deliver training on safe lifting techniques and biomechanics for return-to-work readiness (e.g., safely handling 50+ lbs)
• •Collaborate with multidisciplinary teams including disability management specialists and prosthesis manufacturers for seamless RTW transitions

Working within an interprofessional acute care team to provide physiotherapy services across General Surgery, Orthopedics, and Neurosurgery units. Responsibilities include ensuring patients meet acute rehabilitation goals in preparation for discharge, providing patient and family education on conditions and treatment plans, and modifying plans of care as needed—particularly for elective surgery patients. Care is delivered in accordance with professional and organizational standards using evidence-based practice to optimize patient outcomes.

• •Collaborate with interprofessional teams including physicians, nurses, and allied health professionals for comprehensive patient care
• •Cover General Surgery, Orthopedics, and Neurosurgery units ensuring acute rehab goals are met for timely discharge
• •Identify and address educational needs for patients and families regarding condition and treatment plans

Delivering geriatric acute care physiotherapy in accordance with professional standards of practice, including comprehensive assessment, treatment, and patient education. This role involves safely mobilizing medically complex patients (non-weight-bearing, lines/tubes, high acuity), implementing chest physiotherapy techniques, and developing treatment plans based on clinical findings. Additional responsibilities include providing educational programs for hospital staff (e.g., safe transfer training for nurses), supervising Rehabilitation Assistants, and contributing to evidence-based care planning aligned with organizational strategic direction.
